Criminal Activity Disclosure Policy


IUPUI is committed to maintaining a safe environment for all members of the university community. As part of this commitment, the University requires applicants that who have 1) been subject to formal disciplinary action (including, for example, but not limited to, suspension or expulsion) for academic or non-academic reasons at any high school, postsecondary institution, college, or university? or (2) have been charged or convicted of a crime (or a foreign legal equivalent) that has not been expunged by a court? or (3) have any currently pending criminal charges (or a foreign legal equivalent) against you or (4) have engaged in any behavior that caused injury to any person(s) or property (including, for example, but not limited to, vandalism or behavior that led to a restraining order against you) which resulted in some form of discipline or intervention to disclose this information as a mandatory step in the application process. A previous conviction or previous disciplinary action for non-academic reasons at any high school, post-secondary institution, college, or university does not automatically bar admission to the University, but does require review. To help expedite this review, please fill complete the form below return it along with your disclosure statement by certified mail as soon as possible to our office. Please note, faxed copies will not be accepted.

A complete disclosure includes the following:

  1. A complete Undergraduate Admissions Application.
  2. All required transcripts.
  3. A complete Prior Conduct Information and Release form
  4. A complete written statement (Click here for more information).

> The Prior Conduct Information and Release form and written statement must be mailed or hand delivered to Undergraduate Admissions.

All documentation must be received by the posted deadline dates.

Please note, there is a minimum timeframe of 4 to 6 weeks for reviewing criminal activity disclosures once it is received by the Criminal Activity Review Committee.
