Criminal Activity Disclosure Policy


IUPUI is committed to maintaining a safe environment for all members of the university community. As part of this commitment, the University requires applicants who have (1) been arrested or convicted of any felony or a misdemeanor such as simple battery or other convictions for behavior that resulted in injury to a person(s) or personal property or (2) who have been subject to formal disciplinary action for non-academic reasons at any high school, post-secondary institution, college, or university attended to disclose this information as a mandatory step in the application process. A previous conviction or previous disciplinary action for non-academic reasons at any high school, post-secondary institution, college, or university does not automatically bar admission to the University, but does require review.

A complete disclosure includes the following:

  1. A complete Undergraduate Admissions Application.
  2. All required transcripts.
  3. A complete Prior Conduct Information and Release form
  4. A complete written statement (Click here for more information).

> The Prior Conduct Information and Release form and written statement must be mailed or hand delivered to Undergraduate Admissions.

All documentation must be received by the posted deadline dates.

Please note, there is a minimum timeframe of 4 to 6 weeks for reviewing criminal activity disclosures once it is received by the Criminal Activity Review Committee.
